Full credit to milsmani at SD, I'll quote his instructions:

1) On the Pleasant Holidays website click on "Book Now" under "Hawaii Deals from $399"
2) Choose Sheraton Maui from LAX/SFO option
3) Under select a combination choose "Air + car"
4) Choose "United Fly/Drive - LAX & SFO" option
5) Choose dates from Dec 10th to Dec 14th
6) Choose San francisco Airport departure.
7) Choose Kahului Maui arrival airport
8) Select No for Trip cancellation insurance option.
9) The end result $432 for two people with a economy car
